Full Name (of the Policyholder/Proposer)
Contact Details: Valid Email Address, Mobile Number, and Correspondence Address.
Identity Proof: PAN Card or Aadhar (masked) only for KYC verification if required.
We collect the following sensitive information strictly to build your case:
Policy Documents: Policy number, terms & conditions copies.
Dispute Details: Rejection letters, email threads with the insurer, and complaint reference numbers.
Medical Records: Discharge summaries, diagnostic reports, and doctor’s notes (Required ONLY for Health/Life Insurance claims).
Payment Proofs: Premium receipts or cancelled cheques (to prove policy validity).
System Logs: IP Address and Browser type to ensure website security and prevent spam submissions.
